False. Originally, I thought it was something like a magical jumpsuit, which made whoever wore it shrink. (I thought this because of its in-game effect, and I didn't notice the face on it.)

False. I'd prefer a Mr. Radish. (One of my in-progress quests requires a Mr. Radish, and I've completed the quest which requires a Parrot Head. (I have 3 Parrot Heads, and no Mr. Radishes.))
