on the left side of the level. If you look at the top pear on the left side, you'll see that in his little chamber the ceiling is raised somewhat. If you jump up in there from the platform at the right, the extra life will appear. I'd recommend that you do that only once the coast is clear, because those pears are outrageously annoying.
