54.8, how I hate thee.Believe it or not, it gets much easier. The main problem is sensory overload (gotta watch for the saw, the strawberry ball, the lemon seeds, the bananas, the pineapple stomp... and that's just half the screen) -- but eventually your reticular activating system will catch on to the patterns and start screening out the stuff you don't really need to pay attention to at the moment.
Also, FWIW, this is the best strategy I've found so far:
1. Immediately drop all the left-hand enemies onto the bottom floor, but don't kill them yet. Mind the pineapple.
1b. Watch the effect item spawning point with an expression of desperate hope.

2. Stand in the blue bomb-changer, as far out on the ledge as you dare, and kick bombs to the right until the lower right-hand lemon is gone. If you're lucky, you can also take out a banana or two. You should only occasionally need to jump straight up to avoid pineapple stomps.
3. See if you can kill a few bottom-floor enemies without dropping the left strawberry, but if you get him too, just prepare to play some dodgeball. Either way, go through the teleporter when it's clear.
4. Drop the right strawberry from atop the post (where it's easy to dodge left or right), and quickly follow it down to drop it onto the pineapple's level.
4b. By now, probably say hello to Mr. Fire.

5. Time your jump to the bottom so that it also doubles as a ball-dodge. Kill the strawberry (and any lingering bananas), then the pineapple.
6. Finish off the lonely lemon.
There's probably a better approach, but that one has consistently gotten me through with only one or two hats lost. Hope it helps!
